Features and aims of the course

 

The programme prepares Specialist Nurses responsible for overseeing nursing care processes directed at individuals, families, and communities. It is characterised by a strong clinical and specialist focus aimed at the proactive management of chronic conditions and complex care needs within community settings.

Its purpose is to provide preventive and healthcare responses to both expressed and unmet health needs, promoting patient autonomy and facilitating transitions across the different services within the health and social care network.

LEARNING OBJECTIVES

The study plan includes a common core curriculum and specialist subjects:

  • Common Core Area: Clinical leadership, consultancy, and quality improvement.
  • Specialisation:
    • Family and Community Nursing and community resource mapping.
    • Advanced assessment and community triage.
    • Management of chronic conditions, palliative care, and mental health.
    • Health promotion and digital health (telecare and telemonitoring).
  • Clinical Placement (30 ECTS credits): Carried out in Community Hospitals, Community Health Centres (Case della Comunità), Territorial Operations Centres (COT), and home care services.

YOUR FUTURE CAREER OPPORTUNITIES

You will become a Specialist Nurse in Primary and Family/Community Care. You will be able to take on clinical leadership roles within community settings, act as a case manager for complex patients, provide specialist consultancy, and collaborate with local authorities to design support networks and promote social inclusion.
